Requirements:

Certified True Copy of the following:

(Please attach the scanned copy)

  •  Land Title or Land Tax Declaration and Duly Notarized Deed of Sale or any other proof of ownership.
  • 2 Valid Government issued ID with picture & signature.
  • Notarized authorization letter or Special Power of Attorney (SPA) if applying thru a representative.
  • Contract of Lease (if applicable).
  • Proof of relationship to the property owner if the applicant is not the property owner (i.e. birth certificate, marriage certificate etc.).

Additional requirements for special cases:

  • IF OWNER THE LAND/LOT IS ALREADY DECEASED

a. Notarized authorization from heirs.

b. Proof of relationship of heirs (birth certificate, deed of donation etc.).

  • IF TAX DECLARATION SUBMITTED IS FOR THE BUILDING ONLY AND LAND/LOT OWNER IS UNKNOWN

a. Affidavit of undertaking.

  • IF THERE ARE MULTIPLE LAND/LOT OWNERS

a. Notarized authorization from all land owners or Affidavit of Consent.

  • Scanned Copy of 2 Valid Government issued ID with picture & signature of account holder.
